Exeter Chiefs and Saracens came into Saturday sitting in fourth and fifth in the table respectively, knowing a final-day victory would secure the final play-off spot.
Meanwhile, reigning champions Bath and third-placed Leicester Tigers met with the winner securing home advantage in the other play-off encounter.
In the big game of the day at Sandy Park, it was Exeter who booked their spot in the top four after a 32-12 victory over Saracens.
The Chiefs spoiled Mark McCall's final game as Saracens' director of rugby with tries from Max Norey, Henry Slade, Andrea Zambonin and Stephen Varney to continue their season and set up a mouthwatering play-off game away to Bath.
At the Rec, Bath were victorious 24-22 over Leicester thanks to a hat-trick of tries from Thomas du Toit. It means Leicester will make the short trip to table-topping Northampton Saints - who had already booked their postseason berth before Saturday's final round of fixtures - in the opening game of the play-offs.
European qualification was also decided on Saturday. Northampton Saints, Bath, Leicester Tigers, Exeter Chiefs, Saracens, Bristol Bears, Sale Sharks and Gloucester enter the Champions Cup, with ninth-placed Harlequins entering the Challenge Cup.
