Holders Marseille, mostly fielding reserve players, laboured into the quarter-finals when the Ayew brothers combined to score in a 1-0 win at third tier side En Avant Guingamp.
Lyon, French champions seven years in a row from 2002, have been struggling to find their stride domestically this season and lie 14th in Ligue 1 despite claiming a maximum nine points from their opening Champions League matches.
Lyon president Jean-Michel Aulas said he would assess Puel's position after the PSG game and following an embarrassing 1-1 draw at bottom club Arles-Avignon in the league last weekend, the coach's situation does not look good.
It looked as though Lyon would, advance to the last eight of the competition when France striker Jimmy Briand opened the scoring with a cunning lob in the 38th minute.
But PSG levelled with four minutes left when former Lyon midfielder Mathieu Bodmer volleyed home to send the game into extra time.
Although Puel had brought in striker Lisandro Lopez as well as midfielders Michel Bastos and Yoann Gourcuff, Lyon conceded a second goal when Ludovic Giuly benefited from a bad clearance to score from close range in the 101st minute.
Marseille snatched their win at Guingamp against the run of play two minutes before halftime when Jordan powered down the right flank and his cross found elder brother Andre, who headed home from close range.
The brothers, sons of former Marseille player and Ghana international Abedi Pele, almost managed a repeat of the goal in the second half but Andre's header went over the bar.
Marseille defender Stephane Mbia and midfielder Edouard Cisse suffered leg injuries and were replaced in the second half.
Lille hammered 10-man Caen 4-1 and AJ Auxerre thrashed National side Bastia 4-0 after the Corsicans were also reduced to 10 men in the first half.
Quarter-finals draw:
Olympique Marseille v Monaco
AJ Auxerre v St Etienne
Valenciennes v Paris St Germain
Montpellier v Lille
Ties to be played on Nov. 9-10
