The 25-year-old Italian edged out stage two winner Daniele Bennati of Lampre to the line with Australian sprinter Mark Renshaw just behind in third for Crédit Agricole.
None of Gutierrez's GC rivals attempted to overturn the deficit with his Caisse d'Epargne team keeping a lid on proceedings with the latter part of the stage going over the same course used in Milan - San Remo.
The only attack of the day came from Cofidis' Sylvain Chavanel but he was caught just after the Cipressa and a frenetic gallop over the Poggio failed to split the bunch leading to a sprint finish.
